Privacy and Confidentiality:
We collect and maintain personal and medical information about our patients in compliance with HIPAA regulations. We only use and disclose your information for the purposes of providing you with the best possible care, scheduling appointments, and processing payment for services rendered. We maintain physical, electronic, and procedural safeguards to protect your information from unauthorized access, loss, or theft. We do not disclose your information to third parties unless required by law or authorized by you in writing.
Tardiness Policy:
Please arrive 10 minutes prior to your scheduled appointment to allow for time to fill out any necessary forms and prepare for your appointment. Late arrivals will result in a shortened appointment so the next guest will not be delayed, and you will be charged for the full appointment.
Cancellation Policy:
If you need to cancel an appointment, 24-hour notice is required for individual services so other clients can schedule an appointment. At this time the cancellation fee is at 0%.
No Shows:
If you fail to show up for your scheduled appointment without any prior notice (“no show”), you will be considered a “no show” patient.
Prices/Payments:
Although we make every effort to keep our website and spa menu updated, please note that prices and services are subject to change depending on the cost of the products. All services must be paid for at time of service. As all services are elective aesthetic services we are unable to bill medical insurance. Prices of products and services are subject to change. Price changes will not affect service packages that are already purchased. We accept Cash, Credit Cards (Visa, MC, Discover), Apple Pay, Venmo, and Zelle.
Payment Agreement:
By receiving services at ER Wellness PLLC, you acknowledge and agree that you are financially responsible for all charges associated with the services rendered. In the event of default, I agree to pay all costs, including but not limited to, cancellation and no-show fees, collection agencies, and reasonable attorney’s fees. A photocopy of this agreement shall be as valid as the original. Payment of services/products are at the time of the in person visit and telehealth visit. For weight loss medications, payments are collected prior to ordering the medication.
Age Policy:
Clients must be 18 years of age or older.
Right to Refuse Service:
We reserve the right to refuse or discontinue service to anyone that demonstrates inappropriate behavior or refuses to act in accordance with ER Wellness policies.
Scheduling:
To help accommodate your scheduling preferences we suggest booking appointments one week in advance. You can schedule appointments by calling 640-492-0172 or booking on the book now button on ER Wellness Webpage. ER Wellness has flexible scheduling, please contact ER Wellness if an appointment slot is not available to get you scheduled.
Photo Policy:
As a medical facility using state of the art technology, ER Wellness PLLC may take pictures before treatment and at appropriate intervals throughout your treatments. These photos are important for clinical purposes and to track progress. We reserve the right to use these photos to demonstrate pre and post procedure results. They may be used for our training manuals, presentations, website, social media and advertisements. For your privacy no names will be used and eyes will be blacked out if the client chooses. Photos will only be used on social media, websites, and with advertisements with given permission on the consent form.
Privacy Policy:
In order to provide the highest quality service at ER Wellness PLLC, we collect and store personal information regarding your health as it pertains to the services you receive, your contact information (email, address, phone), your payment method and your treatment history. Protecting your privacy is important to us. We use an online Electronic Medical Records service to securely store your data. This is in compliance with privacy laws. We will not share or sell your information to third parties. Your health and service history information is used to facilitate booking your appointments, recommending appropriate treatments and products, follow up phone calls to ensure your satisfaction, promotional emails pertaining to offers that may be of interest to you and surveys to ensure we are continually improving to best suit your needs. Your financial data is stored securely in our payment software using a globally recognized payment processor. We appreciate your business and value you and your privacy.
